SIR Forms — Enumeration Form, Form 6, 7, 8 Guide

The Election Commission utilizes specific forms to add, remove, correct, and verify voter listings. Understand which form to submit and the pathways available for offline and online registrations.

Short answer: During the Special Intensive Revision (SIR), Booth Level Officers deliver an Enumeration Form to each household. If your name is dropped in the subsequent draft roll, submit Form 6 online via the Voter Service Portal or offline to your BLO to restore your registration. Forms 7 and 8 handle objections and corrections.

Verification

Enumeration Form (SIR)

The primary form used during physical house-to-house BLO visits. Pre-filled with existing family details. The head of the household must verify all names, note deaths or permanent migrations, sign, and hand it back to the BLO along with optional documents (like parent-linkage proofs).

Inclusion

Form 6 — New Elector / Restoring Missing Names

Used by citizens turning 18, new residents, or electors whose names were dropped in the draft roll. Submitting Form 6 is the official legal path to get added to the revised voter list. Requires identity, age, and residence proofs.

Objection / Deletion

Form 7 — Objection to Listing / Request for Deletion

Used to object to the inclusion of a name in the draft roll, or request the deletion of an existing name due to death, duplicate registration, or permanent migration. Cannot be processed without serving notice to the affected voter.

Correction & Modification

Form 8 — Correction, Shifting, or Marking PwD

Used for shifting of residence (within or outside assembly constituency), correcting personal particulars (spelling, age, relation), requesting a replacement EPIC card, or marking an elector as Person with Disability (PwD).

How to Submit Your Forms

Online Submissions (Recommended)

Online processing is faster and allows digital tracking.

  • Visit the ECI Voter Service Portal at voters.eci.gov.in .
  • Or download the official Voter Helpline Mobile App (Android / iOS).
  • Register, fill the digital form, upload scanned proofs, and receive a tracking reference ID.

Offline Submissions

Ideal for those lacking stable internet connectivity.

  • Hand physical forms directly to your Booth Level Officer (BLO) during home visits.
  • Or submit at local Voter Information Centers or the office of the Electoral Registration Officer (ERO).
  • Always obtain a signed and dated physical acknowledgment receipt from the receiving officer.

The Appeal Ladder & Helpline

If your claim under Form 6 is rejected, or if you believe your name was deleted without proper notice and reasoned order, you have a statutory right to appeal:

  1. First Appeal: File an appeal with the District Magistrate (DM) / District Election Officer (DEO) within 15 days of the order.
  2. Second Appeal: If unsatisfied, file a second appeal with the state Chief Electoral Officer (CEO).
